Mail requests for Grant County criminal history records can be made by filling out the Request for Conviction Criminal History Form. This form should be sent by mail to the address above.

Mail service payments can be made with a check, money order, or a Bank Card Authorization Form (for Visa, Mastercard, and American Express credit/debit cards). A name-based background check costs $32.
